How to Obtain Your Ohio Medical Cannabis Card

Your 3-Step Application Process
Step 1: Consult with a Certified Physician
Begin by scheduling an appointment with an Ohio-licensed doctor who is approved by the state’s Cannabis Control Program. During your consultation, the physician will evaluate your condition to determine if it qualifies for medical cannabis treatment. If approved, they will:
- Create your profile in the Patient & Caregiver Registry.
- Provide a recommendation that outlines specific products and dosage guidelines based on your needs.
What to bring:
- A government-issued ID
- Proof of Ohio residency (alternative forms of identification are acceptable if you do not have an Ohio driver’s license; refer to state guidelines for details).

Step 2: Activate Your Online Account
Once your physician has registered your details with the state, you will receive an activation email. Click the link provided to set up your account on the Medical Cannabis Control Program’s website. During activation, double-check that your:
- Full name
- Date of birth
- Residential address
- Contact number
is accurately entered. This information will be used during every visit to a dispensary, so accuracy is essential.
Step 3: Pay the Registration Fee and Download Your Card
After verifying your information, you’ll be prompted to pay a nominal registration fee—just one penny. Once processed, you will receive a link to download your digital medical cannabis card. Note that Ohio now issues digital cards only; there’s no physical card.

Renewing Your Card
Medical cannabis certifications in Ohio must be renewed annually. To renew, simply obtain a new recommendation from a state-certified physician and update your registry account accordingly.
